Sagittal sectioning through the adult female bladder wall brings the ureteric orifice into profile at the posteroinferior aspect of the trigone, where the distal ureter pierces the detrusor and runs obliquely within the intramural tunnel before opening into the bladder lumen. The mucosa lines the lumen and reflects over the orifice as a small ridge, while the detrusor muscle forms the thick middle layer external to the submucosa. Posterior to the bladder base, the relationship to the anterior vaginal wall and cervix is typically close, with the ureter coursing lateral to the vaginal fornix as it approaches the bladder. That oblique intramural course is the anatomic basis of the ureterovesical junction anti-reflux mechanism, a teaching point that becomes clinically concrete in vesicoureteral reflux, recurrent pyelonephritis, and postoperative reflux after ureteral reimplantation. Shortening or kinking of this segment also matters in iatrogenic injury, since the distal ureter and ureteric orifice can be distorted during hysterectomy, anterior colporrhaphy, or pelvic radiation.
