Description
Sagittal sectioning through the adult female bladder brings the trigone into profile, with the ureter entering the bladder wall obliquely to form the ureteric orifice on the posteroinferior aspect of the lumen. The intramural (submucosal) ureter courses anteroinferiorly toward the bladder base before opening, while the bladder neck and proximal urethra lie inferior and anterior to the trigonal region. Posteriorly, the vesicouterine pouch and anterior vaginal wall are typically closest, and the ureteric tunnel sits lateral to the midline but is best appreciated in sagittal relation to the bladder outlet. Key landmarks. That oblique ureteric passage matters because it acts as the physiologic antireflux mechanism, compressed during detrusor contraction to limit vesicoureteral reflux.
