Description

Anteriorly, the pubic symphysis and retropubic space lie in front of the urinary bladder, while posteriorly the cervix and anterior vaginal wall approximate the bladder base in the adult female pelvis. Within the bladder lumen, the trigone forms a smooth, triangular area on the posteroinferior wall, bounded superolaterally by the right and left ureteric orifices and inferiorly by the internal urethral orifice leading into the short female urethra. In sagittal section the bladder apex trends superiorly toward the peritoneal reflection, while the bladder neck sits inferiorly and slightly posterior, continuous with the proximal urethra. The trigone matters because it anchors the vesicoureteral junctions and concentrates pathology that clinicians evaluate endoscopically, not least the relationship of the interureteric ridge to ureteric jets and reflux. Small shifts in the bladder base during pregnancy, pelvic organ prolapse, or after hysterectomy can alter ureteral angulation, a setup for obstruction or iatrogenic ureteric injury. Smooth mucosa.
