Description
Sagittal section through the adult female pelvis centers on the urinary bladder, with its dome (apex) positioned anterosuperiorly and the bladder neck tapering posteroinferiorly into the proximal urethra. The trigone occupies the posteroinferior bladder base, bounded by the ureteric orifices superiorly and the internal urethral orifice inferiorly, while the posterior bladder wall lies immediately anterior to the cervix and anterior vaginal wall. Anteriorly, the retropubic space (space of Retzius) separates the bladder from the pubic symphysis, and superiorly the peritoneal reflection forms the vesicouterine pouch between bladder and uterus. This midline perspective matters because it clarifies the female bladder’s support and its relationship to the uterus and vagina, the anatomy that underlies stress urinary incontinence and cystocele on pelvic exam and imaging. Surgeons rely on these planes during retropubic midurethral sling placement, Burch colposuspension, and abdominal hysterectomy, where dissection too close to the bladder base risks cystotomy near the trigone and ureteric orifices.
