Description
Anterior pelvic anatomy centers on the adult female urinary bladder (vesica urinaria), with the bladder neck tapering inferiorly to the internal urethral orifice (ostium urethrae internum) at the start of the urethra. Posteroinferiorly, the trigone forms a smooth triangular zone between the right and left ureteric orifices and the internal urethral opening at its apex. Superiorly the bladder dome expands toward the peritoneal reflection, while the urethra courses inferiorly and slightly anteriorly from the bladder base, remaining short and in close relationship to the anterior vaginal wall. Clear identification of the internal urethral opening matters because it defines the bladder outlet, the landmark most clinicians mean when they describe “bladder neck” function and obstruction. In female stress urinary incontinence, hypermobility of the urethrovesical junction and bladder neck descent during increases in intra-abdominal pressure are central teaching concepts, and this orifice becomes the reference point for continence mechanisms and for positioning during anti-incontinence surgery. It also frames discussion of iatrogenic injury: urethral catheterization, cystoscopy entry, and periurethral dissection all begin at this junction.
