Description
Arising on the visceral surface of the right hepatic lobe, the right portal fissure (fissura portalis dextra) is presented in dorsal (posterior) view as a deep cleft extending superiorly toward the posterior margin of the liver. The fissure lies lateral to the porta hepatis and medial to the posteroinferior contour of the right lobe, with the caudate region positioned more medially and superiorly in the same posterior field. Posterior relations with the inferior vena cava groove and hepatic venous outflow territory are implied by the orientation of the dorsal surface. A sharp septal landmark. Recognizing the posterior extent of the right portal fissure matters when you correlate surface anatomy with internal portal segmentation, because this cleft marks a plane that can correspond to intersegmental boundaries used in Couinaud-based planning. During hepatectomy or segment-oriented resection, surgeons often rely on external fissures and sulci on the visceral and posterior surfaces to anticipate where portal pedicles and hepatic veins will course, and to avoid misidentifying a fissure for a vascular groove near the caval impression. This dorsal perspective also supports teaching around anatomic variability, since accessory fissures on the right lobe can mimic pathologic clefts on CT or ultrasound and lead to false positives for laceration or intrahepatic collections.
