Description

Giardia duodenalis is rendered in its trophozoite form with the classic bilaterally symmetric, pear or tear drop outline, a broad anterior end tapering posteriorly. Paired nuclei sit anteriorly and medially, giving the characteristic “face-like” appearance, while longitudinal axonemes and the median bodies are suggested within the translucent cytoplasm. Multiple flagella arise in pairs and trail posteriorly, with ventral and posterolateral bundles positioned to support motility and attachment. Contrast lighting against a black field accentuates the ventral concavity where the adhesive (ventral) disc would be located. Cellular morphology matters in giardiasis because the trophozoite, not the cyst, is the attaching and multiplying stage in the proximal small intestine, classically the duodenum and jejunum, where it contributes to malabsorption and steatorrhea. That ventral disc is the teaching landmark for pathogenesis: the organism apposes the brush border rather than invading, so symptoms often reflect disruption of epithelial function and microvillus architecture instead of frank ulceration.
