Description
Anterior to the vagina and inferior to the peritoneal reflection, the female urinary bladder is lined by urothelium (transitional epithelium) forming a folded mucosa that becomes smoother as the lumen distends. Toward the base, the trigone mucosa spans between the ureteric orifices and the internal urethral orifice, a region that remains comparatively smooth and firmly adherent to the underlying detrusor. Beneath the epithelium, the lamina propria supports the mucosal surface and merges deeper with the muscularis propria of the bladder wall. Orientation centers on the luminal surface. Urothelial morphology matters because it reflects both function and disease: umbrella cells and their glycosaminoglycan surface layer help maintain a urine-blood barrier, and disruption correlates with painful bladder syndrome (interstitial cystitis). For pathology teaching, the bladder mucosa is the origin site for most urothelial carcinomas, which commonly present as papillary lesions or flat carcinoma in situ, often sampled via cystoscopy and transurethral resection. Trigonal anatomy also anchors clinical discussions of vesicoureteral reflux and postoperative edema near the ureteric orifices.
