Description
Sagittal section through the adult female urinary bladder centers the vesical lumen and its mucosal lining, with the urothelium (transitional epithelium) forming rugae that flatten toward the distended dome. Superiorly the bladder apex projects toward the anterior abdominal wall, while the base (fundus) lies posteriorly and continues inferiorly into the bladder neck and proximal urethra. Posteroinferiorly, the trigone forms a smoother mucosal field between the ureteric orifices and the internal urethral orifice, contrasting with the corrugated mucosa of the bladder body. Anterior relationships favor the pubic symphysis, while posteriorly the bladder abuts the anterior vaginal wall and cervix region in typical female pelvic anatomy. A sagittal mucosal view is the one clinicians reach for when explaining why most of the bladder wall looks folded on cystoscopy but the trigone does not. That distinction matters in practice: chronic catheterization, recurrent cystitis, or bladder outlet obstruction can alter rugal patterning and mucosal vascularity, and carcinoma in situ often presents as a subtle, flat urothelial lesion rather than an exophytic mass. Orientation along the bladder neck also supports teaching continence mechanisms, where the internal urethral orifice and proximal urethra sit inferior to the trigone and anterior to the vagina, a frequent route of iatrogenic injury during pelvic surgery.
