Description
Seen from the inferior aspect of the male liver, the transverse fissure at the porta hepatis lies between the quadrate lobe anteriorly and the caudate lobe posteriorly, with the right and left lobes extending laterally on either side. At the hilum, the portal vein occupies a posterior position relative to the hepatic artery proper and the common hepatic duct, reflecting the typical arrangement within the hepatoduodenal ligament. The groove for the inferior vena cava sits posterior and slightly right of midline, while the gallbladder fossa and the ligamentum teres fissure fall anterior and to the right and left of the porta, respectively. Orientation at the porta hepatis matters because it dictates safe dissection during hepatobiliary and transplant surgery, where controlled inflow occlusion (the Pringle maneuver) targets the portal triad at the free edge of the lesser omentum. Misidentifying the common hepatic duct versus an arterial branch can lead to bile duct injury during cholecystectomy or hilar dissection, and variant hepatic arterial anatomy often declares itself at this level. Small margins, big consequences. This inferior perspective also reinforces how portal venous inflow and biliary drainage converge at the hilum before branching within the liver.
