Description
Seen from the left lateral aspect with a posterior bias, the left main (primary) bronchus courses inferior to the arch of the aorta and posterior to the pulmonary artery as it enters the root of the lung. Within the hilum, the bronchial tube lies posterior to the pulmonary veins, then divides into lobar bronchi that angle laterally and inferiorly toward the lung parenchyma. Proximally, the tracheobronchial bifurcation would be implied at the carina, with the left main bronchus taking a longer, more horizontal route than its right-sided counterpart. Bronchial branching stands out. Orientation of the bronchi relative to the hilar vessels matters in thoracic surgery and bronchoscopy, because the bronchus is the firm, cartilaginous landmark when veins collapse and arteries blend into surrounding tissue. A left lateral view also supports teaching of why aspirated foreign bodies more often enter the right main bronchus, while the longer left main bronchus is more affected by extrinsic compression from enlarged left hilar lymph nodes or adjacent aortic pathology. Lung cancer staging hinges on it.
