Description
Arising from the aortic arch, the left common carotid artery ascends in the root of the neck within the carotid sheath, lying medial to the internal jugular vein and anterior to the cervical sympathetic trunk. Superiorly it courses deep to the sternocleidomastoid, then divides at the level of the upper border of the thyroid cartilage into the internal carotid artery posterolaterally and the external carotid artery anteromedially. Small cervical branches are typically absent from the common carotid, keeping attention on the clean vertical trajectory toward the head. Clean anatomy. Clear landmarks. Left-sided presentation matters because the left common carotid has a direct aortic origin, a longer intrathoracic segment, and different patterns of atherosclerotic plaque compared with the right, which usually arises from the brachiocephalic trunk. This view supports procedural planning for carotid endarterectomy and carotid artery stenting, where plaque at the bifurcation and the relationship to the vagus nerve in the sheath guide dissection planes and neurovascular risk. It also aligns with common Doppler ultrasound teaching, where identifying the carotid bulb and differentiating internal from external carotid flow patterns depend on recognizing the bifurcation level and branch anatomy.
