Description
Seen from a posterior neck perspective, the common carotid artery ascends within the carotid sheath, positioned anterolateral to the cervical vertebral bodies and medial to the internal jugular vein. Superiorly, the vessel approaches the carotid bifurcation at approximately the C4 level, where the internal carotid artery continues posterolaterally toward the skull base while the external carotid artery remains more anteromedial in the carotid triangle. Deep to the sternocleidomastoid, the artery lies adjacent to the prevertebral fascia and longus capitis and is crossed obliquely by the omohyoid belly in the lower neck. Orientation matters here. A posterior contour emphasizes the carotid artery’s relationship to the transverse processes, sympathetic trunk, and lower cranial nerve corridor near the skull base, relationships that are easy to flatten or misread in purely lateral teaching views. Surgeons exploit this anatomy during carotid endarterectomy, where the internal carotid must be controlled without traction injury to the hypoglossal nerve and where plaque at the bifurcation can distort the usual vessel geometry. The same spatial logic applies when interpreting CTA or MRA in neck rotation, where apparent kinking or tortuosity often reflects the artery’s deep course rather than a focal stenosis.
