Description
Arising from the right upper lobe bronchus, the apical segmental bronchus (B1) courses superiorly toward the lung apex, positioned cranial to the posterior (B2) and anterior (B3) segmental bronchi. From a right-sided perspective, the branching point is appreciated near the superior aspect of the hilum, where the upper lobe bronchus diverges from the bronchus intermedius. Proximal bronchial cartilage rings and the bronchial lumen are emphasized, with the segmental airway oriented toward the apical segment of the right upper lobe. Airway geometry is the story here. This anatomy matters in bronchoscopy and thoracic surgery because B1 is a frequent target for segment-directed sampling, and its takeoff angle can complicate navigation compared with B2 and B3. Accurate identification supports interpretation of right upper lobe atelectasis patterns and helps localize endobronchial lesions or aspirated material that tracks into segmental bronchi. In VATS segmentectomy of the apical segment, surgeons rely on precise recognition of the B1 origin to avoid injuring adjacent segmental branches and to plan stapling lines that match the bronchovascular anatomy.
