Description
Arising from the right ventricle, the pulmonary trunk (truncus pulmonalis) courses superiorly and slightly leftward before dividing into the right and left pulmonary arteries at its bifurcation. From an anterior perspective, the left pulmonary artery runs more horizontally toward the left hilum, while the right pulmonary artery passes to the right, typically traveling posterior to the ascending aorta and superior vena cava to reach the right lung. The proximal segment of the bifurcation sits inferior to the aortic arch and anterior to the left main bronchus, a tight relationship that explains why small shifts in perspective can change what you read as anterior or posterior in this region. Landmarks matter. This sectional arrangement at the pulmonary trunk bifurcation is the key teaching view for understanding pulmonary outflow and the start of low pressure pulmonary arterial circulation, and it maps directly to what clinicians interpret on CT angiography when tracing a saddle pulmonary embolus across the main pulmonary artery into both branches. Cardiothoracic surgeons also rely on this anatomy during pulmonary artery cannulation and during mobilization of the great vessels, where the left recurrent laryngeal nerve loops under the aortic arch near the ligamentum arteriosum adjacent to the left pulmonary artery.
