Description
Arising from the right ventricle, the root of the pulmonary trunk is presented in anterior (frontal) orientation, centered on the infundibulum (conus arteriosus) and the pulmonary valve at the base of the great vessel. Superior to the valve, the pulmonary trunk ascends and angles slightly posteriorly as it approaches the level where it will bifurcate into right and left pulmonary arteries. Anteriorly, the arterial cone forms the muscular outflow portion of the ventricular wall, while the valve ring sits at the junction between ventricular myocardium and the elastic arterial wall. No peripheral pulmonary vessels are carried into the field, keeping attention on the radix trunci pulmonalis and proximal course. Stripping away the surrounding blood vessels directs the eye to the surgical and imaging landmarks that matter at the pulmonary root: the valve annulus, the infundibular outflow tract, and the proximal trunk where congenital narrowing and postoperative distortion often concentrate. This is the anatomy you correlate with pulmonary valve stenosis and tetralogy of Fallot repair, where residual right ventricular outflow tract obstruction can occur at the infundibulum or just above the valve (supravalvular). It also maps cleanly to echocardiographic assessment of the right ventricular outflow tract and main pulmonary artery in parasternal short axis views.
