Description
Centered in the anterior cranial fossa, the ethmoid bone is presented from an endocranial perspective with the calvaria removed, bringing the cribriform plate into clear view. Numerous foramina perforate the plate for olfactory nerve filaments, while the crista galli rises in the midline as a superior projection for attachment of the falx cerebri. Laterally, the cribriform plate is bordered by the orbital plates of the frontal bone and the foveae ethmoidales, and posterior to it the floor steps up toward the planum of the sphenoid and the margins of the lesser wings. Suture lines along the frontal bone and adjacent cranial vault provide orientation within the cranial cavity. This angle matters when you need to teach or plan around the anterior skull base, where the thin ethmoid roof forms a common pathway for cerebrospinal fluid rhinorrhea after trauma or endoscopic sinus surgery. A small defect at the cribriform plate or along the lateral lamella can produce a leak and set the stage for ascending meningitis, while fractures in this region also explain post-traumatic anosmia. The crista galli and its relationship to the midline is also a practical landmark when discussing falx attachment and imaging correlation in the anterior cranial fossa. A fragile area.
