Description
Arising from the posterior division of the mandibular nerve (CN V3), the meningeal branch (nervus spinosus) is shown coursing superiorly from the infratemporal region and re-entering the cranial cavity through the foramen spinosum, classically alongside the middle meningeal artery. Its fibers distribute on the endocranial surface of the dura mater in the middle cranial fossa, tracking anteromedially across the floor of the temporal region toward dural territories near the trigeminal impression and sphenoid. Yellow peripheral nerve elements contrast against the light gray cerebral surface, with the temporal lobe region positioned superior and lateral to the expected dural course. Small structure, big implications. Clinically, this branch matters because it carries sensory afferents from supratentorial dura, a major substrate for headache referral patterns through the trigeminal system. Irritation of the dura by middle meningeal artery dilation, dural inflammation, or extra-axial blood can drive pain transmitted via V3 meningeal fibers, and the intimate relationship at the foramen spinosum becomes relevant in skull base trauma or during surgical and endovascular management of the middle meningeal artery. For teaching, it is also the cleanest example of a cranial nerve branch that exits the cranial cavity and then returns to supply the meninges. Neuroanatomy and head and neck anatomy courses use this view to connect the trigeminal ganglion, mandibular nerve branching, and dural innervation in a single mental model, and it fits well in lecture slides on migraine mechanisms, referred pain, and meningeal vascular supply.
