Description
Running along the anterior surface of the interosseous membrane, the anterior interosseous veins course in the deep anterior forearm between the radius laterally and ulna medially. Proximally they track with the anterior interosseous artery and the anterior interosseous nerve (branch of the median nerve), deep to flexor digitorum profundus and adjacent to pronator quadratus near the distal forearm. Superficial venous pathways are also shown, with the cephalic vein laterally and the basilic vein medially communicating toward the brachial veins in the arm. Orientation is anatomical, anterior view, with the forearm supinated and the palm facing anteriorly. Clinically, these deep venae comitantes matter because they explain why forearm swelling, pain, or bleeding can be missed when attention stays on the cephalic and basilic veins, and why deep venous thrombosis of the upper limb can involve paired interosseous channels. The close companionship of the anterior interosseous veins with the anterior interosseous nerve places them in the same surgical corridor used in volar approaches to the radius and in exposures for pronator quadratus elevation, where careful retraction and hemostasis protect both nerve and vessels. Deep and easy to underestimate.
