Description
Running on the anterior surface of the male heart, the right coronary artery courses in the right atrioventricular (coronary) sulcus from the aortic root toward the acute margin, while the left main coronary artery divides into the anterior interventricular artery (left anterior descending, LAD) in the anterior interventricular sulcus and the circumflex artery in the left atrioventricular groove. Diagonal branches typically leave the LAD obliquely to supply the anterolateral left ventricle, and right marginal branches track inferiorly along the right ventricular free wall. Superiorly, the ascending aorta and pulmonary trunk frame the coronary origins at the aortic sinuses. Green lymphatic channels and nodes overlay the epicardium, aligning with the coronary grooves and great vessels. A rare pairing. This anterior perspective matches the surface anatomy cardiologists think in during coronary angiography and CT coronary angiogram interpretation, where the LAD and its diagonals define the classic distribution of anterior wall ischemia. It also supports operative orientation in coronary artery bypass grafting, when the left internal thoracic artery is commonly anastomosed to the LAD and saphenous vein grafts may target diagonal or right marginal branches.
