Description
Medial thigh anatomy is centered on the adductor brevis, positioned deep to adductor longus and superficial to adductor magnus as it spans from the body and inferior ramus of the pubis to the proximal linea aspera of the femur. Proximally, its fascicles run inferolaterally from the pubic origin toward the posterior aspect of the femoral shaft, with a short, broad tendon blending into the femoral attachment. Just posterolateral to the pubic origin, obturator externus occupies the deep plane near the obturator foramen, placing the adductor brevis immediately anterior to key deep hip rotators and the anterior hip capsule. The acetabulum, femoral head and neck, and adjacent pubic rami provide the osseous frame for these relationships. Understanding this layer matters when you need to orient yourself around the obturator nerve’s anterior division, which typically courses between adductor longus and adductor brevis, while the posterior division passes between adductor brevis and adductor magnus. That plane is a familiar source of confusion in dissection and a common site for entrapment-related medial thigh pain, and it also explains why obturator neuropathy can present with adductor weakness and gait instability. Adductor brevis is also a practical landmark when working from the anteromedial approach to the hip or when interpreting groin pain that overlaps adductor tendinopathy and athletic pubalgia patterns.
