Description
Positioned on the medial side of the proximal thigh, the adductor brevis is presented in lateral profile with the pelvis and femur as bony reference. Its broad origin tracks along the body and inferior ramus of the pubis, then the muscle belly runs posteroinferolaterally to a short, flattened tendon inserting on the proximal linea aspera and pectineal line of the femur, just inferior to the lesser trochanter. Superficial to it sits the adductor longus, while the adductor magnus lies deeper and more posterior, creating a layered adductor compartment relationship that is easy to appreciate from the side. Bony landmarks, including the acetabulum, femoral head, greater trochanter, and lesser trochanter, frame the muscle’s proximal attachments. Lateral visualization of adductor brevis matters because the muscle is often overlooked clinically, yet it sits close to structures targeted in anterior and anterolateral hip approaches and in obturator nerve evaluation. Groin pain labeled as an adductor strain commonly involves the adductor longus myotendinous junction, but tenderness deep to longus and pain with resisted hip adduction can implicate adductor brevis, sometimes alongside iliopsoas and pectineus depending on hip position. Depth cues also help clarify why obturator neuropathy can present with weak adduction despite an intact adductor longus, given the shared innervation patterns across the adductor group.
