Description
Seen from the lateral aspect, the L2 vertebra sits between the L1 and L3 vertebral bodies with an intervertebral disc interposed, its large anterior vertebral body transitioning posteriorly into the pedicles and laminae that frame the vertebral foramen. Superiorly and inferiorly, the articular processes project from the vertebral arch, and the superior articular facets are oriented to face medially, aligning with the sagittal-plane bias of the lumbar zygapophysial joints. Posterior elements, including the spinous process and transverse process, extend dorsally and laterally as levers for paraspinal musculature. A clear lumbar lordosis is maintained across the segment. A lateral view of L2 is often the fastest way to teach load-bearing anatomy and segmental motion in the mid-lumbar spine, where flexion and extension dominate and axial rotation is constrained by facet orientation. Those medially facing superior articular facets matter when you discuss facet-mediated low back pain, degenerative spondylolisthesis patterns (more common at L4 to L5 but explained by the same joint geometry), and why a pars interarticularis defect can destabilize the posterior elements under repetitive extension. This is a mechanical segment. It fails in predictable ways. Course directors can drop this into gross anatomy labs, biomechanics lectures, and radiology teaching files when correlating lateral lumbar radiographs with bony landmarks such as vertebral bodies, posterior elements, and disc height. Spine surgeons and pain clinicians will also find it useful for patient-facing explanations of disc degeneration versus facet arthropathy and for orienting learners before approaches that use fluoroscopic lateral landmarks for medial branch blocks.
