Description
Prominent in lateral profile, the third lumbar vertebra (L3) sits superior to L4 and inferior to L2, its kidney-shaped vertebral body forming the anterior weight-bearing column and presenting superior and inferior endplates for the adjacent intervertebral discs. Posterior to the body, the pedicles lead into the vertebral arch and spinous process, while the superior and inferior articular processes project to form the zygapophysial (facet) joints with neighboring vertebrae. The transverse processes extend laterally, and the pars interarticularis bridges the articular pillars. Orientation is clear. L3 is a practical reference level in lumbar biomechanics and in daily clinical localization: the conus medullaris typically ends around L1 to L2, so attention shifts at L3 to cauda equina anatomy and to degenerative disease affecting discs and facets. Disc height loss or posterolateral herniation at L3–L4 can narrow the lateral recess and contribute to L4 radiculopathy, while facet arthrosis at the L3–L4 zygapophysial joint can drive axial low back pain and segmental stiffness. A clean lateral view also helps communicate how anterior vertebral body wedging and endplate irregularity contribute to lumbar lordosis changes and osteoporotic compression fracture patterns. Use this illustration in gross anatomy and musculoskeletal modules when teaching vertebral regional features and facet orientation, and in radiology or spine surgery texts to pair with lateral lumbar radiographs and sagittal CT or MRI slices.
