Description
Arising at the thoracolumbar junction, the L1 vertebra is presented from a posterior perspective within an otherwise continuous vertebral column, so the lumbar spinous processes form a midline ridge from inferior thoracic levels down toward the sacrum. L1’s spinous process appears short, broad, and hatchet-shaped, projecting posteriorly from the laminae, with the paired transverse processes extending laterally and the superior articular processes sitting posteromedial to the pedicles. Immediately superior lies T12 with a more caudally sloping spinous process, while L2 sits inferiorly with similar lumbar morphology and increasing vertebral body mass. Blue highlighting isolates two vertebral levels for quick orientation against the surrounding cervical, thoracic, lumbar, and sacral segments. Posterior bony landmarks at L1 matter because they anchor surface anatomy and guide needle and surgical trajectories at the thoracolumbar junction. This level is a common transition zone in compression fractures after axial load, and it is frequently referenced when reporting burst fractures that can compromise the spinal canal through retropulsion of vertebral body fragments. Pedicle screw placement and posterior decompression depend on consistent recognition of the spinous process, lamina, and pedicles, while avoiding iatrogenic violation of the facet joints. Small mistakes here have consequences. Orthopedic and neurosurgical teaching files can pair this view with CT or fluoroscopic images to reinforce level identification, posterior element anatomy, and common fracture patterns at T12 to L2.
