Posterior Perspective of the Third Lumbar Vertebra

Posteriorly, the third lumbar vertebra (L3) is centered by a short, broad spinous process projecting in the midline from the junction of the laminae. On either side, the laminae sweep laterally toward the transverse processes, while the superior articular processes rise just lateral to the base of the spinous process and carry prominent mammillary processes on their posterior aspect. Inferior articular processes sit caudal to the pars interarticularis, framing the posterior margin of the vertebral arch around the vertebral canal. Thick pedicles and the superior and inferior vertebral notches define the intervertebral foramen boundaries at this level. L3 matters because posterior bony landmarks guide both diagnosis and approach in the lumbar spine. The pars interarticularis between the superior and inferior articular processes is the weak link in spondylolysis, a defect that can be subtle clinically but is easiest to conceptualize on a posterior view where the facet complex and laminae are read as a single ring. Facet joint orientation in the lumbar region also explains why L3 to L4 pain generators often include zygapophysial arthropathy, and why medial branch blocks and radiofrequency ablation target nerves that course along the junction of the transverse process and superior articular process, near the mammillary process.
