Description
Positioned at the cervicothoracic junction, the C7 vertebra is rendered in posterior view with its characteristically long, nonbifid spinous process projecting inferiorly in the midline. Lateral to the spinous process, the laminae converge toward the vertebral arch, and the paired transverse processes extend laterally toward the scalene attachments. Superior and inferior articular processes border the vertebral foramen, aligning with the adjacent cervical vertebrae to form the posterior elements of the spinal column. Two vertebrae are color coded in blue to draw attention to a specific level within the stacked cervical segment. Clinically, this posterior perspective matters because C7 is the vertebra prominens, the most reliable palpable cervical landmark for counting vertebral levels in surface anatomy and for planning imaging or intervention. Misidentifying C7 can shift procedures by a level, which is a practical issue during fluoroscopy-guided cervical epidural injection, posterior cervical approaches, or when correlating dermatomal symptoms with suspected C7 radiculopathy from foraminal stenosis. The long spinous process also anchors the nuchal ligament and provides a reference for midline posterior exposure.
