Description
Running on the anterolateral thigh, the descending branch of the lateral femoral circumflex artery leaves the profunda femoris region and tracks inferiorly between rectus femoris (medial) and vastus lateralis (lateral), heading toward the lateral border of the patella. Proximally, the femoral artery lies medial within the femoral triangle and continues distally into the adductor canal before becoming the popliteal artery behind the knee, while the highlighted descending branch stays anterior and lateral as it approaches the genicular level. Around the knee, the vessel contributes to the periarticular network by joining genicular branches from the popliteal system in a patellar (genicular) anastomosis. Side-by-side deep veins accompany the arterial trunks, with more superficial venous channels implied in the subcutaneous plane. A clean, anterior map. This anterior projection matters because the descending branch is a dependable pedicle for anterolateral thigh (ALT) perforator flaps and vastus lateralis muscle flaps, where surgeons plan perforator location and caliber relative to rectus femoris and the lateral intermuscular septum. Patellar and distal femur blood supply also depends on the integrity of this genicular anastomosis, a point that becomes practical in knee arthrotomy approaches, distal femoral fractures, and in collateral flow when the superficial femoral artery is stenosed or occluded. Getting the spatial relationship right prevents wrong-plane dissection.
