Facial Artery

Description

Originating from the external carotid artery in the carotid triangle, the facial artery courses superiorly and anteriorly deep to the posterior belly of digastric and stylohyoid, then passes through or adjacent to the submandibular gland before winding around the inferior border of the mandible at the anteroinferior margin of the masseter. From that palpable “facial pulse” point it runs obliquely across the cheek in a characteristically tortuous path, traveling toward the angle of the eye as the angular artery near the medial canthus. Along its ascent it lies deep to the mimetic musculature, with portions related to the buccinator and orbicularis oris, and it is shown in context with neighboring veins and the layered facial musculature of the male head and neck. Understanding this course matters because the facial artery is both a surgical landmark and a common source of brisk bleeding in facial lacerations, especially near the mandible and the nasolabial fold where it becomes more superficial. The terminal angular artery anastomoses with the dorsal nasal branch of the ophthalmic artery, a key extracranial to intracranial connection that underlies the spread of infection from the “danger triangle” of the face and informs safe injection planes in aesthetic practice. Tortuosity also explains why the vessel can be mobile yet vulnerable during elevation of nasolabial and cheek flaps.
