Description

Running obliquely across the lateral face, the facial vein is traced from the perioral region toward the inferior border of the mandible, then into the submandibular triangle where it continues toward the internal jugular venous system. Along its course it lies superficial to the buccinator and masseter region and anterior to the parotid gland, with the mandible and zygomatic arch forming firm lateral landmarks deep to the vessel. Posteriorly, the sternocleidomastoid and the angle of the mandible frame the transition from face to neck. Clear orientation. Venous anatomy on the cheek matters because the facial vein participates in clinically relevant connections between superficial facial drainage and deeper pathways, including communications with the angular vein near the medial canthus and the superior ophthalmic vein, a route implicated in cavernous sinus thrombosis from infections of the “danger triangle” of the face. For procedural planning, the vessel’s relationship to the marginal mandibular branch of the facial nerve at the inferior border of the mandible is a recurring point of discussion during submandibular approaches, incision placement, and flap elevation. Surface landmarks make it teachable.
