Description
Posteriorly, a long, nonbifid spinous process identifies the vertebra prominens (C7) at the cervicothoracic junction, with its laminae converging to form the vertebral arch behind a stout vertebral body. Laterally, the transverse processes project toward the scalene region, and the transverse foramina, typically smaller than in C3 through C6, sit anterolateral to the pedicles. Inferior articular facets orient to meet T1, while the superior facets remain aligned with the lower cervical column. Landmarks read clearly in lateral stacking. C7 matters because it is the palpable transition point between mobile cervical lordosis and the stiffer thoracic kyphosis, and its morphology guides both counting and instrument placement when surface landmarks are unreliable. Clinically, the C7 to T1 level is a common corridor for posterior cervicothoracic fixation, where pedicle size, lateral mass geometry, and facet orientation determine screw trajectories and the risk profile for the C8 nerve root and vertebral artery. Misidentifying C7 as T1 can shift an entire surgical level. It happens.
