Description
Presented is a male os coxae in medial perspective with the sacrum removed, so the internal contours of the ilium, ischium, and pubis read clearly as a single innominate bone. Superiorly, the iliac fossa occupies most of the medial ilium, bounded posteriorly by the auricular surface and iliac tuberosity where the sacroiliac joint and its interosseous ligaments normally anchor. Inferior and anterior, the pubic body and superior pubic ramus approach the pubic symphysis medially, while posterior and inferior elements of the ischium curve toward the ischial spine and tuberosity. Medial views of the hip bone matter because they highlight the articular and ligamentous interfaces that drive pelvic ring stability, not just the acetabulum. The auricular surface and adjacent roughened iliac tuberosity provide the bony substrate for the sacroiliac ligaments implicated in postpartum pelvic girdle pain and in the inflammatory erosions and sclerosis seen in sacroiliitis. Osseous landmarks around the obturator foramen and ischial spine also orient trainees to the course of the pudendal neurovascular bundle as it wraps around the ischial spine near the sacrospinous ligament, a consideration during pelvic floor surgery and pudendal nerve blocks.
