Centered within the popliteal fossa, the popliteal vein ascends posterior to the knee joint as a large-caliber blue vessel, receiving paired venae comitantes from the anterior and posterior tibial veins distal to the joint line. Superficial to it, the small saphenous vein tracks proximally in the midline before draining into the popliteal vein, while the tibial nerve lies more superficial and slightly lateral in the fat of the fossa. Deep and slightly anterior to the vein, the popliteal artery occupies the deepest plane against the posterior capsule. Relationships matter here. Framing structures include the semimembranosus and semitendinosus on the superomedial border and the biceps femoris on the superolateral border, with the medial and lateral heads of gastrocnemius forming the inferior margins; their tendons and the posterior cruciate ligament region sit anterior to the neurovascular bundle.
