Description
Arising from the radial artery as it courses across the dorsolateral carpus, the princeps pollicis artery runs distally along the first metacarpal toward the thumb, accompanied by superficial dorsal veins that converge proximally toward the wrist. From an anteriorly oriented presentation of the hand vasculature, the vessel’s track is appreciated relative to the thenar region, lying lateral to the first dorsal interosseous muscle and giving digital branches that continue along the margins of the pollex. Smaller arterioles radiate into the thumb pulp and nail bed territories, while venous channels return blood along parallel, more superficial paths. Thumb perfusion is unforgiving. Disruption of the princeps pollicis artery, whether from laceration near the first web space, iatrogenic injury during thenar surgery, or spasm and thrombosis after radial artery cannulation, can threaten distal tissue viability and complicate wound healing in the pollex. For teaching, this view also clarifies why the radial artery contributes so heavily to the deep palmar system and why collateral flow between the princeps pollicis artery and the radialis indicis artery matters when planning flap design or assessing ischemia.
