Description
Anteriorly, the superficial cervical venous network converges toward the right and left external jugular veins as they descend superficial to the sternocleidomastoid muscle and angle inferiorly toward the subclavian veins behind the clavicles. Deeper in the carotid sheath, the internal jugular veins run vertically from the jugular foramina to join the subclavian veins at the venous angles, forming the brachiocephalic veins in the superior mediastinum. Tributaries expected in this projection include the facial, retromandibular, and anterior jugular veins, with transverse anastomoses near the jugular venous arch just superior to the manubrium. Relation matters: the common carotid arteries course medial to the internal jugular veins, a tight pairing in the lower cervical region. Central venous access lives and dies by these relationships. The internal jugular approach for a central line targets the vein anterolateral to the carotid pulse at the level of the cricoid cartilage, while the subclavian approach tracks under the clavicle where pneumothorax risk rises and compression of bleeding is limited. Jugular venous distension and waveform assessment also depend on understanding which vein you are looking at, the external jugular can be misleading, while the right internal jugular more faithfully reflects right atrial pressure in heart failure, tamponade, and tension pneumothorax.
