Description
Oriented from an inferior perspective, the pia mater forms a thin, glossy meningeal layer tightly adherent to the ventral surface of the brainstem and the inferior cerebellar hemispheres, draping into the sulci and around the cerebellar folia. Arterial channels course within the pial bed over the medulla oblongata and pons, with paired vertebral arteries converging toward the basilar artery along the midline, and cerebellar branches fanning laterally toward the cerebellar cortex. Blue venous pathways track back toward posterior fossa dural sinuses at the periphery, while yellow cranial nerves emerge from the pontomedullary junction and preolivary and postolivary sulci, running anteriorly and laterally in close relationship to the pial vasculature. Nothing sits between pia and neural tissue. This inferior pial view matters because it is where neurovascular relationships are least forgiving: cranial nerves and small perforators share tight corridors at the skull base. Surgeons working around the clivus, foramen magnum, and cerebellopontine angle rely on these surface landmarks to protect the basilar trunk and vertebral artery branches while identifying nerves implicated in posterior circulation stroke syndromes and lower cranial neuropathies. Teaching the distinction between pia mater and the more superficial arachnoid and dura also becomes practical here, where arachnoid cisterns can obscure vessels on imaging even though the pial vessels remain fixed to the brain surface. Ideal uses include neuroanatomy and head and neck anatomy courses covering meninges, cranial nerves, and posterior fossa circulation, and for neurosurgical atlases illustrating approaches to the ventral brainstem and cerebellum. It also fits neuroradiology teaching files that correlate inferior brain surface anatomy with vertebrobasilar aneurysms, PICA territory infarcts, or CPA mass effect on cranial nerves.
