Description
Viewed from the inferior aspect, the arachnoid mater forms a translucent sleeve over the ventral surfaces of the cerebral hemispheres and bridges across the basal cisterns, separating from the pia mater where cerebrospinal fluid accumulates. Anteriorly, the arachnoid spans the region of the olfactory tracts and optic nerves toward the optic chiasm, then continues posteriorly over the interpeduncular fossa between the cerebral peduncles. Inferior to the diencephalic floor, the membrane drapes around the pituitary stalk region and extends across the pons and medulla oblongata, where multiple cranial nerves emerge laterally from the brainstem. Fine arachnoid trabeculae are implied where the membrane lifts away from the brain to create cisternal spaces. An inferior meningeal view matters because the basal arachnoid is where clinical anatomy becomes three dimensional: subarachnoid cisterns, cranial nerve root entry zones, and vascular corridors all occupy the same confined space. This is the perspective you need when teaching subarachnoid hemorrhage patterns, where blood layers dependently in basal cisterns, or when explaining why inflammation in meningitis can compromise cranial nerve function at the skull base. For neurosurgical orientation, the relationship of the arachnoid to the interpeduncular and prepontine cisterns frames common microsurgical dissection planes around the basilar artery and oculomotor nerve.
