Description
Cut through the testis, the thick tunica albuginea forms the outer fibrous capsule, with septa testis projecting inward from its deep surface to partition the parenchyma into wedge-shaped lobuli testis. Those septa converge centrally toward the mediastinum testis, where the rete testis resides and where efferent ductules will exit superiorly to the head of the epididymis. Within each lobule, you would expect tightly coiled seminiferous tubules arranged deep to the capsule, with interstitial (Leydig) cells occupying the connective tissue between tubules. Orientation is typically described with the epididymal border posterolateral and the mediastinum along the posterior aspect. This sectional anatomy matters because many testicular disease processes and operative steps respect, distort, or traverse these fibrous planes. A firm, hypoechoic intratesticular mass on ultrasound is interpreted against the normal lobular architecture, while segmental infarction or focal orchitis can present as a wedge-shaped lesion that mirrors lobular boundaries and their vascular supply. During radical inguinal orchiectomy for suspected germ cell tumor, surgeons avoid scrotal violation and handle the testis with its coverings intact, but teaching often returns to this cut surface to explain how a tumor expands within and across septa toward the mediastinum and rete.
