Description
Seen just deep to the thin auricular skin, the helicis major (major helical muscle) runs as a narrow band along the anterior margin of the helix of the pinna, paralleling the curved rim from superior to inferior. Its fibers lie superficial to the perichondrium of the auricular cartilage, with adjacent auricular muscles and facial expression muscles near the root of the auricle where the pinna blends into the temporal scalp. Medial to the helix, the contour of the scapha and antihelix is implied by the underlying cartilage, while the lobule remains inferior and lacks cartilage. For teaching, this is the cleanest way to explain why auricular muscles are usually functionally minor in humans yet anatomically consistent. The helicis major matters in otoplasty and auricular reconstruction because subdermal dissection over the helix can violate these delicate muscle slips and their small neurovascular supply, contributing to postoperative contour irregularities or skin edge compromise. It also clarifies the plane you want for local flaps and for placement of perichondrial sutures when reshaping the helix.
