Description
Running just deep to the dermis along the superior helix margin, the musculus helicis major appears as a narrow, superficial slip on the lateral surface of the auricle (pinna). From an anterior, oblique facial profile, it lies anterior to the scapha and superior to the crus helicis, following the curvature of the helical rim toward the upper pole of the external ear. Adjacent facial soft tissues frame the auricle, with regional mimetic musculature over the temporal and preauricular area providing spatial context. Small anatomy. Easy to miss. Recognition of the helicis major matters because it sits in the same surgical corridor used for otoplasty and other auricular cartilage procedures, where precise subdermal dissection can preserve or deliberately release small auricular muscles to control skin tension and contour at the helical rim. This view also helps clarify why most people have minimal voluntary ear movement, the intrinsic auricular muscles (including helicis major) are thin, variably developed, and often functionally overshadowed by the extrinsic auricular muscles and surrounding scalp fascia. When planning incisions along the helix for skin cancer excision and reconstruction, the relationship between dermis, perichondrium, and these delicate muscular fibers can influence flap mobility and postoperative rim definition.
