Description
Seen from the dorsal aspect, the superficial dorsal venous network spreads across the dorsum of the foot, coalescing into the dorsal venous arch just distal to the ankle. Medially, tributaries course anterior to the medial malleolus to continue as the great saphenous vein along the medial leg. Laterally, companion channels pass anterior to the lateral malleolus toward the small saphenous vein, while digital veins drain proximally from the toes into the metatarsal veins overlying the extensor tendons. Dorsal pedal venous anatomy matters because this is where surface landmarks guide cannulation and where superficial reflux patterns become visible in chronic venous disease. The relationship of the dorsal venous arch to the malleoli helps distinguish saphenous tributaries from deeper venous outflow pathways during duplex ultrasound mapping and preoperative marking for varicose vein treatment. Misidentification is common. Clear depiction of ankle-crossing veins also supports discussion of why edema and stasis changes often begin distally and why perforator incompetence around the ankle can drive venous hypertension in the foot.
