Description

Arcing across the dorsum of the right midfoot, the dorsal venous arch (arcus venosus dorsalis pedis) collects superficial tributaries from the dorsal metatarsal veins and drains proximally toward the anterior aspect of the ankle. Medially, flow continues into the great saphenous vein anterior to the medial malleolus, while laterally the arch feeds the small saphenous vein as it courses posterior to the lateral malleolus. Superficial veins run within the subcutaneous tissue over the extensor tendons, with the network fanning distally toward the toes and converging proximally into larger-caliber channels. Orientation is clear. For teaching venous return in the lower limb, a dorsal pedal view clarifies a common point of confusion: the dorsal venous arch is superficial, but it communicates with the deep system through perforating veins and with the dorsalis pedis venae comitantes near the ankle. Clinically, this is the territory inspected in chronic venous disease, where ankle and dorsal foot edema, varicosities, and skin changes can track along the great saphenous pathway, and it is also where venous access is sometimes attempted when upper extremity options are limited. The malleoli matter.
