Description
Arcing along the lateral border of the male foot, the lateral marginal vein collects tributaries from the dorsolateral forefoot and courses proximally toward the heel, where it continues as the small saphenous vein posterior to the lateral malleolus. Dorsal digital veins and a dorsal venous arch are suggested on the dorsum, with perforating channels passing plantarly and deeper to communicate with the plantar venous plexus. At the ankle, the superficial veins lie superficial to the extensor retinacula and peroneal (fibularis) tendons, while deeper drainage tracks toward the posterior tibial and fibular venae comitantes in the retromalleolar region. Relationships are shown in true lateral orientation, with the calcaneus posterior and the fifth metatarsal and cuboid forming the lateral column distally. Lateral pedal venous anatomy matters because it is where clinicians often access or injure superficial veins, intentionally or not. Cannulation at the dorsal venous network and near the lateral malleolus is common when upper-limb access fails, but the small saphenous territory also overlaps pathways for superficial thrombophlebitis and the varicosities that can accompany small saphenous valve incompetence. Wound healing at the lateral heel and ankle also depends on intact venous outflow, a point that becomes concrete in chronic venous insufficiency and in postoperative edema after hindfoot or ankle procedures. Simple surface anatomy, real consequences. Use this asset in gross anatomy and vascular modules to teach superficial versus deep venous drainage of the foot, and in podiatry, orthopedic, or vascular surgery publications discussing lateral ankle approaches, venous mapping, or peripheral IV access.
