Running along the lateral border of the male foot, superficial venous channels course posterior to the lateral malleolus before ascending the distal leg as the small saphenous vein, a trajectory that parallels the sural nerve in the posterolateral compartment. Distally, veins from the lateral marginal vein drain the dorsolateral foot, communicating with the dorsal venous arch over the metatarsals and with plantar venous tributaries near the cuboid and fifth metatarsal. Beneath the venous network, the fibula lies lateral to the tibia, and the talus and calcaneus form the hindfoot, with tarsals, metatarsals, and phalanges arranged from proximal to distal in clear lateral profile. A lateral perspective helps clarify how superficial venous return from the foot reaches the popliteal fossa, and it makes the relationship between the small saphenous vein and the lateral ankle landmarks easy to teach. This is the territory encountered during lateral malleolar incisions, endoscopic sural nerve procedures, and harvest or cannulation of superficial veins, where inadvertent injury can lead to troublesome bleeding or postoperative sensory disturbance along the lateral foot.
