Description
Seen from the lateral aspect of the male head and neck, the transverse facial artery branches from the superficial temporal artery within the parotid region and tracks anteriorly across the masseter muscle. Its course sits slightly superior to the parotid (Stensen) duct as both traverse the cheek, with the zygomatic arch and lateral maxilla providing bony landmarks superiorly and anteriorly. Deeper in the neck, the common and external carotid arteries ascend medial to the mandible, while the internal jugular vein descends posterolateral to the carotid sheath contents; smaller facial and scalp vessels arborize over the temporal region. For clinicians, this relationship of artery to parotid duct is the point. Parotidectomy, duct repair after cheek lacerations, and preauricular approaches to the temporomandibular joint all cross a corridor where bleeding from the transverse facial artery can be brisk and where injury can accompany ductal disruption or sialocele formation. Facial filler injections and flap elevation over the zygomaticomasseteric region also benefit from a clear mental map of this vessel’s superficial plane relative to the parotid fascia and masseter.
