Description
Seen from a lateral, near-sagittal perspective, the ventricular system is traced through the cerebral hemisphere with emphasis on the C-shaped sweep of the lateral ventricle. The anterior (frontal) horn extends superior and anterior to the head of the caudate nucleus, while the body of the ventricle arches posteriorly above the thalamus. Inferiorly, the temporal horn curves into the temporal lobe, running lateral to the hippocampal formation, and the atrium sits posterolateral to the pulvinar region. Cerebrospinal fluid spaces read clearly against the surrounding parenchyma. That side-on geometry matters when teaching why deep gray structures wrap around the ventricles and how pathology follows those contours. Enlargement of the lateral ventricles in hydrocephalus, or ex vacuo dilatation in cerebral atrophy, is often first appreciated by the increased caliber of the frontal horn and temporal horn on midline imaging. For procedural anatomy, the relationship of the caudate nucleus and thalamus to the ventricular wall frames the risks of intraventricular hemorrhage extension and guides ventricular catheter trajectories, which aim for the frontal horn to avoid the basal ganglia and internal capsule.
