Description
Presented in lateral profile on a sagittal brain section, the ventricular system is seen as the C shaped lateral ventricle with its body deep to the corpus callosum, the atrium (trigone) posteriorly, and the temporal horn curving inferiorly and anteriorly within the temporal lobe. Superiorly, the corpus callosum arches over the ventricular cavity, while the septum pellucidum and fornix form medial boundaries toward the midline. Centrally, the third ventricle sits between the thalami, narrowing inferiorly toward the hypothalamus and continuing into the cerebral aqueduct; posterior fossa relationships are anchored by the fourth ventricle between the brainstem and cerebellum. Spatial logic is clear. This lateral orientation matters when you need to connect ventricular anatomy to the way pathology distorts it. Temporal horn enlargement is an early and practical marker in obstructive hydrocephalus patterns and in ex vacuo dilatation with medial temporal atrophy, and it also explains why mass effect from temporal lobe tumors can efface the horn before midline shift becomes dramatic. The proximity of the foramen of Monro to the fornix and thalamus provides context for memory disturbance after intraventricular procedures and for the risks of trajectory planning during endoscopic third ventriculostomy or ventricular catheter placement.
