Description
Running inferoanteriorly from the lower eight ribs, the external oblique (musculus obliquus externus abdominis) forms the most superficial muscular layer of the lateral abdominal wall, its digitations interlocking with the serratus anterior along the mid to lower thoracic cage. Superiorly, the serratus anterior originates from the lateral ribs and wraps posteriorly toward the medial border of the scapula, while the external oblique fibers descend toward the iliac crest and the anterior abdominal aponeurosis. A lateral, just-beneath-the-surface treatment also brings the bony framework into register: rib shafts and costal cartilages anteriorly, the scapula and proximal humerus at the shoulder girdle, and the lateral ilium and acetabular region inferiorly. Fiber direction reads clearly. Teaching the lateral torso is often where students finally understand how the “sawtooth” serratus anterior and the “hands-in-pockets” external oblique share rib attachments yet drive different biomechanics, scapular protraction and upward rotation versus trunk rotation and forced expiration. That interface also sits on common pain generators: rib fractures and intercostal nerve irritation at the midaxillary line, and external oblique strains in baseball, tennis, and rowing where eccentric control during rotation concentrates load near the costal attachments and iliac crest. Surgeons care about these planes as well, since splitting lateral abdominal wall fibers and respecting their aponeurotic transitions affects exposure and postoperative abdominal wall function, including at the inguinal region where the external oblique aponeurosis contributes to the inguinal ligament.
