Description
Beneath a semi-transparent integument, the anterolateral abdominal wall is dominated by the external oblique muscle (musculus obliquus externus abdominis), its fibers running inferomedially from the lateral thoracic wall toward the linea alba and the anterior iliac crest. Superiorly, the digitations along ribs 5 to 12 interdigitate with serratus anterior and latissimus dorsi, while inferiorly the aponeurosis contributes to the rectus sheath and the inguinal ligament between the anterior superior iliac spine and pubic tubercle. Medial to the muscular belly, the rectus abdominis is implied by the aponeurotic sheet, and the iliac crest forms the inferior bony boundary for the lateral attachment. A clear fiber-direction study. External oblique anatomy matters because its aponeurosis defines key surgical planes and hernia landmarks, including the superficial inguinal ring in the aponeurosis and the relationship to the inguinal canal contents during open inguinal hernia repair. Sports medicine also lives here: strain at the costal attachments or along the iliac crest is a common source of flank and groin pain in throwing athletes, and the fiber orientation explains pain provoked by trunk rotation and contralateral side-bending.
