Description
Broad sheets of the external oblique (musculus obliquus externus abdominis) dominate the anterolateral abdominal wall beneath the skin, their fibers running inferomedially from the lower ribs toward the midline. Superiorly, the digitations interlock with serratus anterior along the lateral thorax, while the inferior borders taper toward the iliac crest and pubic region. Medially, the external oblique aponeurosis contributes to the anterior rectus sheath and approaches the linea alba, and inferiorly it thickens as the inguinal ligament extending from the anterior superior iliac spine to the pubic tubercle. Portions of the rib cage and sternum remain visible as bony landmarks for these attachments, with pectoralis major occupying the anterior chest above. Fiber direction is the teaching point here, because it predicts both action and what you meet surgically in layered abdominal wall dissection. Follow the inferomedial fibers and you are also following the roof of the inguinal canal, where weakness in the posterior wall and enlargement of the superficial inguinal ring can manifest as inguinal hernia patterns that clinicians must distinguish on exam and during repair. Strain injuries in rotational sports often involve the external oblique near the costal attachments, and this view clarifies why pain can localize to the lower ribs or along the iliac crest.
